NUTRITIONAL CONSULTING INFORMED CONSENT AND DISCLAIMER FORM

PLEASE READ THE FOLLOWING TERMS AND CONDITIONS CAREFULLY

Fit My Detox adheres to the following terms and conditions of use. By accessing and using the Site, you agree, on your own behalf and on behalf of any corporation or other legal entity that you represent or for which you access the Site (your "Company"), to be bound by and comply with the following terms and conditions of use (these "Terms") and to comply with all applicable laws and regulations. If you do not agree to these Terms, you are not authorized to use the Site.

GOAL: The basic goal of FitMyDetox is to encourage clients to become knowledgeable about and responsible for their own health, and to help them to reach a personal, optimum level of healthiness through nutrition and supplement related education. Regarding dietary changes and, if warranted, the introduction of nutritional supplements to your daily routine.

Nutritional advice does not provide medical diagnoses, and no comment or suggestions should be construed as being a medical diagnosis. Fitmydetox will provide nutritional advice only, and will NOT be providing any medical advice of any kind whatsoever. FitMyDetox cannot guarantee any specific result or outcome from my suggestions given.

Nutritional advice will be based on information provided by the responsible recipient.

HEALTH CONCERNS: If you suffer from a medical or pathological condition, you need to consult with an appropriate healthcare provider. Consulting with FitMyDetox is not a substitute for being treated by your primary-care provider or other appropriate healthcare practitioners. Marla Feingold is not trained or licensed to diagnose or treat pathological conditions, illnesses, injuries, or diseases.

It is the clients responsibility to provide information if the client has certain medical conditions, illnesses, injuries or diseases that may afflict you. Nutritional advice is no substitute for medical care and is only a complement to the care you receive elsewhere. If you are under the care of another healthcare provider, particularly if you are currently taking prescriptions as a part of such care, it is vitally important that you promptly contact your other healthcare provider(s) and alert them to your use of nutritional supplements. FitMyDetox cannot be responsible in any way for such communication. Nutritional changes you may choose to undertake and the use of supplements in connection there with may be a beneficial adjunct to more traditional care, and it may also alter your need for or the required dosage of your medication. It is the client party’s responsibility to keep your physician informed of changes in your nutritional program prior to starting and supplement or dietary recommendations or modifications. Only your physician can make adjustments to prescription medications.

If you are using medications of any kind, you are required to provide which medications you are using the “prescriptions” area of the your personal information forms in the questionnaire section. Any changes in medications or healthcare while following a Nutritional program or supplement recommendations are reported to FitMyDetox immediately via emailed or in the provider contact webpage. Any potential interactions between medications and nutritional products should be discussed with your pharmacist. If you have any physical or emotional reaction to nutritional therapy, discontinue use immediately and contact FitMyDetox.

COMMUNICATION:

Every client is an individual, and it is not possible to determine in advance how your system will react to the supplements you need and the dietary changes that may be suggested. It is sometimes necessary to adjust your program.

INSURANCE:

Nutritional consulting and the costs associated with supplements may not be covered by insurance and all costs are the sole responsibility of the client. FitMyDetox will not be submitting any reimbursement requests or have any contact with any insurance company for any reason.

WAIVER OF JURY TRIAL:

You hereby knowing and voluntarily waive the right to a jury trial in any actions, suits or proceedings arising out of or relating to these terms and matters contemplated hereby.

By Accessing and using the site, you acknowledge that you have read, understand, and agree to be bound by these terms of use applicable to you.

AGREEMENTS:

By checking the agreement box on the Questionnaire (the client user of FitMyDetox) confirm that I have read and fully understand the above disclaimer, am/are in complete agreement there with and do freely and without duress sign and consent to all terms, conditions and admonitions contained herein, that I am at least 18yrs old or older, the information I have provided is accurate. Moreover, I (the client) agree to indemnify and hold harmless FitMyDetox and Marla Feingold from and against any loss or cost of any kindnwhatsoever relating to my/our execution of this document and the terms contained herein.
